Consent and Acknowledgements

Your consent and acknowledgement for each of the following is a requirement for form submission.

Rate is $900 up to 50 students, and $18 per student for each additional student.

A $175 fee will be added for each additional setup required. Field Trip Set Up Fee is charged when multiple groups are presented to on the same day at the same location or same organization. Note, the setup fee does *not* apply to the initial set up. It only applies to the 2nd and subsequent setups.

If I select the Italian Pasta option, I understand the rate will be $25/pp rather than $18, with a minimum of $1000 up to 50 students. The additional rate per student is the same at $25.

The Real Food Academy will not provide beverages for this event.

If my location is determined to be 15 miles or more from our location in Miami we will be charged a $3 per mile travel fee (i.e. 17 miles x $3 = $51). Click to see a map and get driving directions, which will show the distance.

The Real Food Academy does not require a deposit to hold the date. However, once a date is reserved, we must receive a written cancellation request within 2 weeks of the reserved date. Otherwise, your school/ camp will be billed a $75 cancellation fee.

I will provide the Real Food Academy a written list of any and all food allergies at least one full day prior to the event.
